Anishnawbe Mushkiki Nurse Practitioner Led Clinic

Anishnawbe Mushkiki Nurse Practitioner Led Clinic has a primary focus on Aboriginal Health, Chronic Disease management and prevention. We opened our doors for patients in December 2011 and celebrated with our grand opening ceremony on Wednesday, May 30th, 2012.

Registration for the Nurse Practitioner-Led Clinic will be done using the following guidelines:

1) You do not have a primary care provider and have been accessing walk-in clinics or the emergency department for your health care needs.

2) You have a chronic disease which requires on-going medications and monitoring by a health care provider (diabetes, asthma, arthritis, hypertension etc.)

3) As a Nurse Practitioner we do not have the authority to prescribe controlled substances, such as narcotics, barbiturates, etc., and therefore may not be able to take on your care for these needs.

In order to be screened for out clinic you must fill out an Intake Form. You can do this:
