 JOB SUMMARY: This position is a central role as a coordinator and navigator to assist clients by providing and implementing a service plan to access a full range of services to include, but not be limited, housing, social and welfare services, basic needs, health, job programs, mental health and substance abuse. This position works closely with the program team to ensure clients are offered culturally relevant avenues for rehabilitation that will reduce recidivism and generally improve their quality of life.

MINIMUM REQUIREMENTS: Bachelor’s Degree in Social Work, Psychology, Counseling, Criminology, related social/behavioral science fieldor commensurate training andexperience. Case management skills experience preferred with excellent verbal, written and interpersonal communications skills required. Demonstrated Cultural Competency with Native American populations, traditions and norms. Must be proficient in using MS Word, Excel, and Power Point and with the internet and email communications; experience with electronic health records a plus. Ability to travel, with other staff, within a large geographical area surrounding communities.

DUTIES & RESPONSIBILITIES:

•Service a caseload of 25-30 active participants, all of which will be adult ex-prisoners.
•Schedule intake and case management appointments for clients referred to program.
•Provide unbiased, objective, and non-judgmental individual case management sessions with all clients at regularly scheduled intervals.
•Develop individual service plans for all clients that identify barriers to successful re-entry.
•Conduct cultural history and assessment at intake including client demographic data.
•Work with Team to assess client’s need for other assessments or services.
•Review and discuss client contract.
•Document all client contact and progress, including time and date, type of contact, outcome and plan of action.
•Identify barriers to client’s ability to gain control of their lives and review with Team for improvements to the services coordination plan.
•Provide or broker services to holistically address client’s needs.
•Assist family members to help with understanding, dealing with and supporting clients.
•Provide regular updates to the Program Team and PAR Director.
•Case Manager will receive clinical direction and guidance from the LADC.
•Assist with coordination of cultural events or activities and attend as needed.
•Complete required documentation and statistical data in a timely manner and electronically provide to the DOC.
•Collaborate with other staff members and participate in multi-disciplinary team meetings.
•Assist Cultural Coordinator with linking clients to community cultural activities and social network, including ceremonial activities and spiritual leaders/counselors.
•Refer clients to community resources or to specialists as necessary.
•Assist in all aspects of the Re-Entry program.
•Commit to the provision of culturally competent services to a diverse client population.
•Provide a positive professional image for UMMHC in carrying out duties.
•Adhere to all applicable regulatory statutes and laws.
•Keep supervisor informed of all exigencies.
•Adhere to all UMMHC policies and procedures.
•Accept other duties upon request.
